Have you ever experienced custom newborn photography before?

Custom portrait photography is MUCH different than what many people are used to (chain studios). It's helpful for us to know if this is your first custom portrait photography experience so we can ensure you understand each step of the process.
Which of my newborn images/poses are your favorite?

It's helpful for us to know what you loved about our work and if there is a certain photo that you really liked so that we can better prepare for your session.

SESSION TERMS & AGREEMENT

LIMITATION OF LIABILITY: I hereby waive, disclaim and release Legacy Photo and Design and its respective owners, members, employees, agents, volunteers and representatives from any and all actions, claims or demands that I may have now or in the future, whether known or unknown, against the LPAD Parties, including without limitation those related to injury to or death of persons, or damage, loss or destruction of property arising out of or in connection with the Session or any products related thereto.

INDEMNIFICATION: I hereby agree to indemnify and hold harmless the LPAD Parties from all liabilities, losses, claims, costs (including attorney’s fees and court costs), damages or expenses of whatever nature that may result from injury to or death of persons, or damage, loss or destruction of property arising out of or in connection with the session or any products related thereto.

MODEL RELEASE

I hereby consent that the photographs taken on this day by Amy Oyler may be used by Legacy Photo and Design.

If a helper is in attendance the day of your session; all images taken by Amy Oyler or assisting photographer can be used for the purpose of display, etsy, portfolio, advertising, competitions, or publication to promote the studio. I further agree that the images can be used without additional compensation to the undersigned. I also declare that I have legal authority to grant these permissions and I accept all responsibilities for such.

I have read the foregoing and fully understand the contents hereof. I represent that I am the parent/guardian of the above named model. For value received, I hereby consent to the foregoing on his/her behalf.
